All are welcome! Your nationality or immigration status do not affect your right to register here. We are a Safe Surgery for everyone in our practice area. Everyone living in England has the right to free care from a GP. Ask reception for an interpreter if you find it difficult to communicate in English. Our receptionists won’t ask you about your immigration status.

Registration for asylum seekers or refugees

Patient Access
Patient Access is a service provided by EMIS that allows you to access your practice online. Patient Access includes a number of optional features, including:
- Checking, booking and cancelling appointments.
- Checking your medication and ordering repeat medication.
- Updating your contact information, including mobile phone number and email address.
- Sending messages to your practice, so avoiding the need for a phone call for routine enquiries. Your practice may not offer all these features.
How do I register?
1. Register with a letter
You can register by firstly visiting your practice in person and requesting a registration letter which will enable you to set up a full account. This needs to be requested when you visit the practice and will require identification. Once you have the letter you can then enter the details from the letter into the registration form.
2. Register without a letter
You can also register for the service online without a letter. This process will check to see if your practice uses Patient Access and create a basic account that allows you to book an appointment.
If you use this option, to get access to the complete features that your practice offers you will need to visit them in person and take ID with you.

Registering with us
You can register online or in person.
In order to register in person, please enquire at reception where you will be given a new patient GMS1 Form and Practice Registration Pack to complete. Please ensure you include your email address and mobile telephone number.
When completed, these forms should be returned to our reception team any time between 8.00am and 6.00pm. Staff will register you with the practice within 3 days and you will then be able to book you an appointment.
We welcome registrations from patients in our area who are currently homeless.

HOW TO REGISTER WITH THE PRACTICE ONLINE?
To register online you can complete your new patient registration online by clicking both the first and second circular tabs titled New Patient Questionnaire Online and New Patient Registration Form GMS 1 online (please ensure you include your email address, mobile telephone number and fully complete both forms). The completed forms are electronically sent to the practice.
The practice aims to complete your registration within 3 working days on receipt of your completed registration forms.
Once you are registered, you will receive a welcome message detailing your named GP and practice information.
Any further documentation required will be emailed to you regarding accessibility preferences e.g. Third party consent, Summary Care Records and National Data-opt Out.
Childrens Registrations
If registering children under the age of 5 years:
A paper registration is required for each child which can be printed off from either the practice website or collected via our Reception Desk. Please bring a copy of the child immunisation history from the red book to reception or email to Audley.healthcentre@nhs.net
Temporary Registration
If you become ill whilst away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact one of our surgeries. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.
